Case Management Form

I received Case Management Report Form after filing with the U.S. District Court April 30, 2010. My case is a Track Two Case of Designation Under Local Rule 3 05. The court attached Case Management Report Form. Since any general question I ask will be interpreted as seeking legal advice can anyone share where I can find information or scenarios how to handle this form before I serve the defendant a copy of my case strategy. I find I can't answer many of the disclosures the form requires simply because the defendant has no idea they are soon being summoned due to the suit filed against them. I further don't know understandably what the defendant's position will be in the discovery process makes it hard for me, Plaintiff to prepare the Case Management Form. Any suggestions where I can research to learn this normal procedure to get more insight how to manage this form would greatly be appreciated.
