What forms may an employer require from employee for intermittent FML

Company has farmed out FML management to a third party concern. At the third party concerns request. I have supplied certification of serious health condition form ( there's ) completed by my health care provider
I have received from the management company , FMLA designation notice with FMLA approved, box Intermittent FMLA leave request has been approved.
I also received an additional letter from the management company stating i have sign and return a intermittent leave agreement signed by self and my (employer)companies supervisor. I can not find any where DOL and numerous web sites that this can be required. Is this second form request legal under FMLA guidelines?
