What should I do about a manager who suddenly depicts me a a non performer after FML

The supervisor has treated me with disdain and has now devised a false review after a year of good performance with not one conversation indicating performance problems. Suddenly after suffering a health probelm and taking FMLA, he presents the review and can answer virtually no questions about specific examples. He also uses comments from co-workers who made them to him in another context, but then skillfully states in the review that they told me about problems upon my return from leave. These are all fabrications, so I am assuming the leave was the problem. What can I do? The supervisor has now witheld my increase and has issued me punitive and difficult projects in short timeframes. I am also the oldest (53)in the department. Help!
