Surgeon is clearing return to work with a modified in office schedule after being on long term disability Can employer refuse a work modification?

I had a significant emergency surgery in July and went on short term disability. Within weeks of the surgery i devolped a complication and was back in the hospital. A month later a follow up ct showed that my emergency surgery repair did not work so i underwent a second traumatic surgery in Oct. I also came to the end of my short term leave while in the hospital in October, and immediately segwayed into long term disability. My employer was aware of all of this during this time. Routine recovery for the Oct surgeries would have taken 12 weeks or until Jan 18. I was left with another complication, possible nerve damage thar has slowed down healing. I had a scope at the end of Dec and have continued on long term. My surgeon is now clearing me to work ft on condition that i work a modified in office schedule, 2 non consecutive days in office, 3 days working from home while i continue to heal. My surgeon provided a letter which my employer now has. As of today, my employer has not yet replied to my request to return to work with the requested modifications. My schedule prior to surgeries was 4 days in and one day working from home. The company does not have a formal wfh policy but they do have several individuals working remotely full time and also others who routinely wfh at will. I am concerned that this will push back or refuse to accomodate my modifications. Can they do this?
