Long term disability cancelled while my husband is on it

My husband is on social security disability and on long term disability from his employer. The long term disability gives us small payments each month along with health insurance for my husband and me. He is also on medicare. Each year the company requires disability verification and up until now it has been on issue. We moved a year ago and the company sent the forms to my husband's doctor. he said that he doesn't do those forms. After my husband spoke to him, he agreed to do it. He never did. Then, when the disability specialist called the doctor, he said that he wasn't going to do the forms. Now we have been cut off of disability income and our health insurance has been cancelled. They sent us a certified letter saying that we can appeal this decision, but the requirements for doing so are insurmountable. Is there anything we can do?
