What should I do if my eployer fails to pay my final check?

I quit my job a few weeks ago. My final paycheck was due on Monday and should have been direct deposited to my bank account. I even contacted my back to verify that they had not received the deposit. They had not. It is now Saturday and I still have not received my final paycheck. I have called the company at least once a day since Monday. I have also sent multiple emails to the party responsible for payroll as well as the owner.

When I notified the company that I had not been paid, I was instructed that I had to fill out a payroll inqury form and that once I did this they would investigate the issue. I let them know that I would not be able to make it to the building and stated that they were aware that I had not received payment. I was again told I needed to fill out the form.

When I spoke with the head of Human Resources (this person also processed the payroll) on Friday, I was told that she could not discuss the matter. She then hung up on me.

What do I need to do to receive payment?

Am I owed any addional money as a result of this issue?

I also have unused sick and vacation time, is my employer obligated to pay me for this as well?
