Should I sign a Performance Improvement Plan if I have a documented mental health condition?

I was put on PIP with several other employees, but I would like to let my employer know that I see a doctor for a anxiety disorder. I have been seeing the doctor for years so back up info shouldnt be a problem. Please let me know your thoughts.
