Can I seek legal action against my employer for substantiating an investigative complaint

I was told by management that an investigative complaint against me was substantiated and now it is reflected in my performance evaluation although mentioned in general terms because I said I was never informed that I was the subject of an investigation. I asked how it could be substantiated if I don't know the allegations against me. I asked if I could see the report. The department leader said he's never seen and said his direct report has it; however in a follow-up meeting there was no mention of the report other than being told this will not be in my personnel files because no disciplinary action was taken. However this still implies the company has the report housed somewhere and that I admitted to the wrongdoings so it will still be detrimental to me. How can I go about resolving this because management kept playing musical chair saying someone else has it when I asked. Can I ask HR to show me the report? How can I ensure this report is destroyed? Can I sue?
