No information on my fmla time before they "vollentarily" resigned me

I was never informed how my company calculated my fmla leave until now when I had to take it due to a bulging disc and a tumor in my head. They tried to tell me I vollentarily resigned because I was not released to return to work by the time my fmla ran out. I asked the head of benifits why I was not explained how it was calculated and her answer was because you have not been in the situation of running out of time before. Isn't that alone a violation?
