My employer has less than 50 employees what are my options as far as a Leave of Absence goes?

My employer has 5 employees, I work 30 hours a week with no benefits. I am stressed to the point I can't perform my job properly. I don't want to get fired for poor performance so I was thinking about taking a leave of absence. This is not entirely work related stress so I'm not trying to file a W/Comp claim. What are my options (if any)?

If your condition is sufficiently serious to constitute a disability under the Fair Employment & Housing Act, you may be entitled to take unpaid leave as an accommodation. You should consult a local employment law attorney.
