Laid off after telling HR that I was applying for short term disability for back surgery

I have had ongoing back issues since 2015 that have gotten worse and I decided I needed back surgery because I can't deal with the pain any longer. HR has known of my condition in 2015, i filed an ADA form with HR January 2016 so that they were aware of my condition. 3 weeks ago I contacted HR that I was going to have surgery at end of September.
On Monday I was laid off for 'poor performance'!
Since they are self-insured I am convinced they laid me off so they wouldn't have to pay for short term disability and cost of surgery.
Do I have a case of ADA discrimination and/or age discrimination (I am 59 and oldest person on the team).
My supervisor sent me many emails saying I was doing a 'great job', which is why I can't believe the reason for laying me off.
I had performance problems last year while I was having bad pain, then getting diagnosed, began treatments, and this year I was told I had 'really improved my performance'. Mainly because injections to back and nerve pain meds had helped tremendously with pain.
6 weeks ago I tripped in the office, twisted my back, and pain is worse than before and 2 of my doctors said I should have surgery, which is why I sent HR an email 3 weeks ago I was going on short term disability for surgery and then I was 'laid off' 4 days ago.
I have been in a state of shock because I now cannot have surgery if I have no job I have no insurance and no income from short term disability. Their 'decision' has doomed me to remain in pain, and who will hire me if I am in pain and have a preexisting condition like this.....help!
