In 2012 I was laid off and received 6 weeks severance pay.

I need to file taxes for that year and my W-2 has the severance amount lumped in with my regular income.
In my state, I can list severance pay as a deduction, but I have to have a statement from the former employer OR it must be shown separately on the W-2.
I've requested the statement from the former employer, but they refuse to provide one.
I think it shouldn't be a problem to provide a simple statement indicating the time frame and severance amount.
What are my options now?
