Used my entire FMLA leave for cancer treatment. Treatment is now finished and my doctor released me to full time without any restrictions per my job description. My employer is stating that I need to fill out ADA paperwork in order to return. I did not sign up for disability. Only short term disability while I had surgery. I don't know what they are asking for. My doctor released me.
