Can I cap how many hours my employee works?

I work as a therapist and recently hired a w2 therapist. I pay them one rate for clinical (seeing clients for therapy), and one rate for other work they provide (emails, to-do tasks, etc). For any non-clinical task, I have them email me to approve time over one hour per week. Am I able to tell them I'll only pay one hour per week, even if there are days where they're in the office in between clients?
