Vacation Pay After Giving Notice - No Formal Policy

I work for a very small medical practice. After two years with no paid time off, instead of an offered raise, I negotiated paid time off. This was in January of last year, and the entire pool of two weeks was immediately available to me. No formal policy was put into place.

I have given notice verbally, but not yet in writing. Paying out my PTO has not been addressed, and I'm afraid he will refuse to pay anything out. I'm also afraid that, if I give notice in writing, and say that X number of my days will be vacation days, he will fire me on the spot.

Up until now we've had a great relationship and I don't want to leave the Doctor/ owner in a bind, but I don't want to leave money on the table either.

I'm looking for advice on the best way to handle this.
