Possible retaliation when HR demands intermittent fmla use?

I've been with my current employer for 3.5 years.
My current boss hired on 9/2015. At that time my hours were 6:30-3:30 am. With those hours I could continue chiropractic care on a weekly basis after work hours.
11/2015 schedule of work hours shifted all folks on our team required to work one 6pm and one 5:30pm shift each week. Then 8-5 the remainder of the days. There are four team members and we have assigned days for these hours. My boss was approving my flex time to still make my 8 hours required for the days I had dr appts.
November 23rd I placed a request to formally move teams as there was a need for a team member on a different team.
12/2015 performance reviews were to be completed with due date for employee side of review to be completed no later than 1/15/2016. 1/8/16 advised my boss my side of required review done. Boss portion due 2/2016.
December 23rd I was informed I'd be moved spring 2016.
January 26th 2016 called into a meeting with my site manger and HR. My boss was off this day. Questioned about my weekly leaving early and why. Informed them I had weekly back care and this was never an issue prior years. Also indicated this was the 1st time this was mentioned to me not even my boss had mentioned this. Advised to apply for intermittent fmla.
With all these moving parts and timing of new boss, team change request and reviews I'm concerned this is a retaliation maneuver. I'm not comfortable completing the fmla intermittent leave request documents. Any guidance would be greatly helpful. Thank you.
