My 401K Plan Administer has failed to execute a withdraw request per the plan rules

My wife has become fully disabled. Per the 401K plan rules she has been terminated from the plan. Also, per the rules she is entitled to a full withdraw of her 401K balance to roll over into Roth of her choosing. We have provided her plan administrator with all of the documentation from both the State and her employer's insurance carrier as evidence of her disability. We have sent 4 formal requests since January2011 and to date none of our requests have been honored.
What legal rights do we have to obtain the withdraw?
