I need help with being terminated while on workman's comp

My job emailed me stating they received my request to extend my leave of absence. I'm on workman's comp/disability. It was stated that employees are not permitted to extend leaves of absence without submitting proof of vaccination or approved exemption and I would be fired on or after January 10, 2022. My extension ended on January 15, 2022 and I thought I was fired. January 28, 2022 I received an email asking for my an extension or a return to work note. If no contact it would be considered job abandonment. I was also threatened a week after I had surgery and was told that I had to show up to work or I would be fired. What to do?
