FMLA interference

I am a salaried employee in TN. I am attempting to get intermittent FMLA or a reasonable accommodation to obtain medical treatment. My company uses a 3rd party administrator that I will call LM. LM is requiring me to sign a release granting open access to my financial and medical records before they will begin to process the request. Part of the excuse is that they are refusing the DOL forms and will not provide theirs. I know that this is not ok, I have reached out to my HR rep who stated that the company would not intervene with LM and that I had to decide as they are not liable for what their administrator does.

My question is what should be my next step?
