Can my doctor contact my employer to tell them I am no longer a patient?

My midwife wrote a doctor's note and filled out the FMLA papers for my maternity leave while under her care. Since then I switched doctors. When I switched my Former midwife contacted my employer to tell them I am no longer a patient. My new doctor has to re-do the forms for my maternity leave. Is this legal? Is the midwife required by law to contact my employer to tell them I am no longer with them? Or does this violate my Right to Privacy?
