Are Employers allowed to request Employees to fill out an Health Status Questionaire?

I have not come across this form before. It is a form that requests Employees to fill out a Health Status Questionaire - The Questionaire is given out after hire date. The questionnaire to assist in preventing needless Workers Comp claims. Is this type of questionairre in compliance?

This type of form or information is not banned, provided it is not used for an illegal purpose. The ADA requires that anyone with a disability that needs accommodation to wait until after at least the hiring decision is made to make sure what is required for an accommodation. If the information gathered is not an excuse to get rid of persons with disabilities, then it will probably be legal.
