What types of FMLA notifications are employers responsible for.

Local HR Manger sent certified letter 4 days after not being release by company Dr stating I was out of FMLA and my job was being filled. I never received a retraction as a couple month later I reached out to corporate office and I was covered and approved under FMLA. Is their a time frame in which companies must notify employees or legal obligation?
