Asked to stay longer after term date given in writing. Unemployment effects?

My wife was told she is being terminated because the medical office she works for needs someone with Medical Assistant qualifications, which she does not have, to fill her role.

She was able to get a letter stating the last day of employment and the reason for termination as described above (not attendance or incompetence.)

The office has been unable to hire someone to fill the role with their newly needed MA skills, and has asked her to stay on indefinitely.

Since she has the termination date in writing, is she required to work beyond that date?

If she refuses to do so, will this disqualify her for unemployment benefits?
