My employer is applying time in their personnel records for me as FMLA that did not qualify for my approved FMLA. What should I do?

My employer is applying hours in their personnel records towards my FMLA that was outside for my approved FMLA and not recorded as FMLA hours with Liberty Mutual. They are also stating several dates that are applied to my FMLA with Libierty Mutual are not covered. There were several occasions where I had more episodes per month then my FMLA allowed. I am OK with taking an occurrence for these dates; however, my employer is now giving me a final warning per their disciplinary policy.
