How does a minor file income taxes if paid mostly cash and never given a W4 form

My 14 year old son worked for several weeks over the summer doing odd jobs at a hotel. He was paid mostly in cash and was also paid by check via the mail when the employer was away. He worked 20-30 hrs a week. He was never given a w4 to fill out when he started . He now received a letter from the employer requesting him to now fill out a W9 as a "vendor." I have no problem declaring the few checks he received but am concerned regarding the cash payments since he never received paycheck stubs and we have no idea how much he actually did receive as cash since his hours varied every week.
Please advise. Is this legal by department of labor guidelines?
