Is my Employer violating FMLA since they haven't responded to my requests for intermittent FMLA?

I am currently on intermittent FMLA leave for a serious health condition; however, my doctor accidentally left part of it off, but included it in an attached form. They added the condition on the employer form and resent it, but my employer would not accept it and said I had to fill out new paperwork for a 2nd condition even though the conditionas are related. They also informed me that they would not send the paperwork until I let my supervisor know that I was applying for a 2nd one. I am trying to follow their instructions and have asked four times for the paperwork, but they have only sent me the doctor's part. When I asked them to advise if I was required to fill out the employee part again, I have not received a response. Also, they have told my supervisor what condition was approved and I have reason to believe they are giving her more information than she needs to know about my health. I have also been docked pay because they claim it was not related to my approved FMLA condition.
