Is it legal for my FMLA to state that I applied for FMLA but the employer really did

I missed 6 days of work (not consecutive days) and received a notice stating that I had applied for FMLA on Nov 19,2018 but I didn't, that is one of the days I was out for illness. I have provided 4 doctors covering each of those 6 days. One of my co-workers missed 3 weeks in a row recently due to illness as well, but did not receive an FMLA notice. Our HR person is trying to tell me I provided 12 doctors notes and missed 12 days (that is false) I made and kept copies of the 4 doctors notes I received for being out. So my main question is if the document states that I applied for FMLA when I really didn't and it was management that applied for me (HR told me that management applied for me) is that legal?
