If my company agrees to a LOA, but does not ask for certification, is it FMLA leave?

I had a medical condition which required my hospitalization. My employer is large enough to be covered by the FMLA. My employer agreed to let me take about one or two months off, but nothing was in writing. They asked for a doctor's note, but no FMLA certification was requested or discussed. I have been out of work for almost two months, and I am ready to go back to work. I have a doctor's note saying I am cleared to work. Was my leave an FMLA leave? If I need FMLA leave later in the year, does this time off count against it? If it's not FMLA leave, can my employer fire me or demote me for missing work?
