Did my employer violate FMLA?

I had called off from (i believe) 9-15 june, 2011; this was a total of four working days. I cited severe depression and anxiety attacks as the reason, but was not informed that the leave fell under FMLA.
furthermore, when i returned to work, i was informed that my schedule had been changed as a result of my "constant" calling off rendering me "unreliable" to work my previous schedule. this led to a downward spiral resulting in a 5-day suspension (16-22 june, 2011), during which time i requested FMLA leave for i had been, within that time frame, diagnosed with bipolar disorder. the leave was granted 22 june, even though my disability certificate states that i was incapacitated from 9 june until 9 july, 2011. were their actions in violation of either ADA or FMLA laws? thank you for your assistance.
