Can someone on Workers' Comp be put on Family Medical Leave

If someone was injured on the job and is receiving Workers Comp can the employer, without notice to the employee, put them on Family Medical Leave. If so, when the 12 weeks for Family Medical Leave have passed and the employee still is not able to return to their job, do they just lose their job, and do they need to pay their medical insurance premiums from that point on (the whole family is covered by the medical insurance).
